Registration With The Practice

If you live in our practice area and would like to register with  us please complete one of our registration forms that are avaliable from reception. You have the right to express a preference of practitioner. However, you will be registering with the practice rather than an individual GP.

You will need to attend the surgery preferably bringing your medical card , although this is not essential.  When you complete the registration documents we need to see two types of ID; one picture proof and one proof of address.

 


If you live within the practice boundary and have your medical card, simply bring it to the surgery and hand it to the receptionist. If you have mislaid it, we can provide you with the necessary form. All new patients are requested to see one of the practice nurses for a general health check.

It is now necessary for you to attend the surgery to register so that all relevant information can be given to you by the receptionist.
